CHECK-IN PRIVACY NOTICE

PRIVACY NOTICE - FOR THE CHECK-IN PROCESS

Pursuant to article 13 item 1 and item 2 of the Resolution of the European Parliament and Council (UE) 2016/679 of 27th April 2016 on protection of individuals in relation to personal data processing, we hereby inform that:

1) the administrator of your personal data is Uzdrowisko Połczyn Grupa PGU S.A. with the registered office in Połczyn-Zdrój at Zdrojowa 6 (KRS: 0000025053, Tax ID: 6720006034, REGON: 00028818800000)

2) the inspector of the personal data protection is Uzdrowisko Połczyn Grupa PGU S.A. Mr Andrzej Kwiatkowski, e-mail: iod@uzdrowisko-polczyn.pl or rodo@uzdrowisko-polczyn.pl;

3) your personal details shall be processed in order to:

Carry out the check-in process and possible claims for outstanding charges or damages
pursuant to:

Article 6 item 1 and b

b) the data processing is necessary to execute the contract, the party of which is a person the data refer to or undertake actions upon the request of the person, who the data relate to, before the contract is signed

4) the recipient of your personal data shall be the IT outsources.

5) your personal data shall not be transferred to a third country/international organisation.

6) your personal details shall be stored for the following period;
- until the removal in accordance with the “right to be forgotten”, unless the Medical Documentation Act applies (in case of therapeutic stays)

7) you have the right to access your personal data, correct them, restrict the processing, limit the processing, transfer the data, submit an objection or remove the data

8) you have the right to submit a claim to the President of The Personal Data Protection Office if you consider that the processing of your personal data regards the infringement of the General Data Protection Regulation of 27th April 2016

9) submitting the personal data is the condition to sign the services agreement.
You are required to submit the data, otherwise the signing of the service agreement shall not be possible.

10) Your personal data shall not be processed automatically.
